A Clinical Study of Periventricular-Intraventricular Hemorrhage in Premature Infants


Abstract
We have experienced 56 cases of periventricular-intraventricular hemorrhage confirmed by routinely studied with serial real-time brain ultrasonography from March 1, 1994 to Feburary 28, 1995 at the neonatal intensive care unit of Il Sin Christian
Hospital.
Their medical records & the results of brain ultrasonography were analized to evaluate incidence, sevenity, timing, clinical finding, outcome, & associated conditions of perventricular-intraventricular hemorrhage.
@ES The results were as follows.
@EN 1) The incidence of PV-IVH in premature infants born weighing less than 2500gm & at less than 36 week's gestation was 53.3%.
2) The incidence of PV-IVH in premature infants born weighing less than 1500gm was 78.5% and less than 32 week's gestation was 74.6%.
3) The severity of PV-IVH was grade l in 17 cases(30.3%), grade 2 in 19 cases(33.9%), grade 2 in 14 cases(25.1%) and grade 4 in 6 cases(10.7%).
4) The mortality of PV-IVH was 39.3% and was 17.6% for grade 1, 47.4% for grade 2,35.7% for grade 3 and 88.3% for grade 4.
5) In view of periventricular echodensity, periventricular leukomalacia was in 17.6%.
6) The clinical findings of PV-IVH were apnea, hypotension, decrease of hematocrit more than 10% during 48 hours, bradycardra and change of muscle tone & activity.
7) The risk factors associated with PV-IVH were gestational age less than 32 week, birth weight less than 150gm, hypercarbia, acidemia, acidemia and NaHCO3 administration during 24 hours after birth, hyaline membrane disease, preumothorax,
patent
ductus arterrosus, sepsis and Ventitaror.
